As a Clinical Lead at Max@Home in Mumbai, your role involves overseeing patient monitoring in high-need areas to ensure consistent high-quality care is provided. Your key responsibilities will include: - Overseeing patient care in high-need areas and ensuring proper monitoring - Coordinating patient assessments by ATL and INH teams - Facilitating stakeholder meetings and building relationships with internal and external doctors - Ensuring readiness of new clinical products like Dialysis at Home and Mother & Child services - Conducting daily clinical team meetings and home visits for patient assessments - Identifying red flags and addressing safety concerns promptly - Organizing and conducting family meetings to update them on patient progress and care plans - Providing counseling and guidance to family members regarding patient care - Maintaining high standards of patient care and identifying areas for quality improvement - Documenting incidents, preparing detailed reports, and facilitating investigations - Monitoring patient care trends and reporting findings through data analysis - Leading daily team meetings and ensuring continuous communication on patient progress - Engaging in case reviews and implementing corrective measures for improved outcomes - Facilitating coordination with doctors and care teams to align treatment plans - Organizing and facilitating training sessions for nursing staff - Coordinating OJT for TL, ATL, nursing supervisors, and nurse educators - Ensuring continuous upskilling for nursing staff and leadership through relevant training modules - Conducting interviews for nursing roles and assisting in the recruitment process - Ensuring staffing requirements are met - Developing, managing, and overseeing palliative care projects and related training modules - Coordinating dashboards to track the progress of palliative care patients - Participating in RCA for incidents affecting patient care and safety - Supporting the development and implementation of CAPA to improve care delivery - Collecting, analyzing, and presenting data on patient care quality - Developing and managing dashboards displaying key performance indicators related to patient care Qualifications required for this role include an MBBS degree and a minimum of 5 years of experience.
